Mixed Selection Order Form

Use the BLW Mixed Selection Order Form to order a variety of wines rather than a straight six or dozen bottles of wine.

  • You may purchase wine in either size so long as there is a minimum order of 6 bottles.
  • If you order 12 bottles or more using the form, you will receive a 5% discount plus freight.
  • When you download the form you will need a PDF reader, (click here to visit the Adobe website to download Adobe Reader).
  • If your suburb is listed in the Zone A table below, the delivery fee is $5 per case*.
  • If you live in Sydney, Wollongong, the Central Coast or the Blue Mountains (based on the range of postcodes in the table below) your wine will be delivered by Australia Post for $13 per case*.
    • If you are unsure of the freight charge it will be calculated for you. Otherwise, give Bob a call on 0410 624 203 or send an inquiry via the Contact Page.
  • Once you have completed the form and signed it, send it to us via email, or alternatively, you may fax the form to (02) 9969 3583.
  • Upon receipt of the order, we will send a confirmation.
  • We will not charge your credit card until we confirm the total cost of your order including freight.
  • Your privacy is important to us and as such, we do not store credit card details. Please refer to our Policies page for more information.
    • As soon as your payment has been processed, the form will be shredded.
